Required Documents
- 2 printed copies of a completed DS-11 passport application form. (Complete the form online but do not sign. Must be signed at an acceptance facility)
- Two passport photos. 2″x 2″ white background. (Photo service is available at any UPS office)
- Original/certified U.S. birth certificate. NOTE: A certified birth certificate has a registrar’s raised, embossed, impressed, or multicolored seal, registrar’s signature, and the Date the certificate was filed with the registrar’s office (which must be within 1 year of your birth), or an expired US passport or Original naturalization certificate.
- Proof of Identity. (a valid driver’s license or government issued photo I.D)
- 2 printed copies of a completed Letter of Authorization. (Our office will email you our letter of authorization once full payment is made)
- Please print 1 copy of your Travel itinerary from the airline, showing your date of travel. (Must be consistent with travel date chosen above)
- A check for $190.00 and a check for $19.53. The checks should be payable to the "US Department of State", on the memo line, applicant DOB and full name is required.
Getting Documents Authorized (If you are not able to physically meet in our office you will need to do this in your current location, please schedule a phone consultation so we can support you in the process)
Please locate your preferred office Acceptance Agent (AA) office and visit the location.
Please ensure that the Acceptance Agent does the following:
- Witnesses the signing of the completed DS-11 application
- Collects $35.00 execution fee
- Checks your ID
- Verifies the documents and seals them into an envelope
- Please inform the acceptance agent that you will be submitting the application through a registered courier working directly with a Passport Agency to expedite the processing and that the sealed documents will be hand delivered to the Department of State. This Department of State policy is called “Hand Carrying of Executed Applications.” The policy is detailed on pages 31 and 44 of the Acceptance Agent Manual.
- The Acceptance Agent will then return the sealed envelope to you. Do not open the envelope. Bring the sealed envelope with the rest of the quick passport documents to your Preferred UPS/Fedex Store for processing.

- A business letter, signed by the applicant, addressed to the passport office, explaining why a second, quick passport is necessary.
- A check for $190.00 (For Adults) or $160.00 (For Minors under 16) and a check for $19.53. The checks should be payable to the "US Department of State", on the memo line, applicant DOB and full name is required.
Your U.S Passport can only be renewed if ALL of the following conditions are met:
- You still have your U.S. Passport in your possession.
- Your previous U.S. Passport was issued when you were age 16 or over.
- Your previous U.S. Passport was issued no more than 15 years ago.
- Your U.S. Passport is undamaged.

Note: Each parent or guardian must submit one of the following:
- Valid Driver’s License
- Valid Official U.S. Military ID
- Valid U.S. Government ID
- Valid U.S. or Foreign Passport with recognizable photo
- Naturalization/Citizenship Certificate from BCIS with recognizable photo
- Alien Resident Card from BCIS
NOTE: Both parents and child need to be present at post office or county clerk when executing your kid’s or children rush passport application. If only one parent can be present, the un-accompanied parent needs to complete a DS-3053 application and have it notarized.
